Community Living Support (CLS)
Our Services:
Community Living Supports (CLS) is a community-based residential service offered through TennCare CHOICES and Employment and Community First CHOICES (ECF CHOICES). This service provides a continuum of support for clients residing in provider-managed homes with 2 to 3 roommates, fostering each resident’s independence and full integration into their community.
Having the right level of CLS service is important. At Deep Springs Home Health, we strive to ensure that each client’s services are based on his or her specific needs and required level of support.
OPTIONS for Community Living is a state-funded program. The program was created to provide the elderly, as well as adults with disabilities, home- and community-based service choices. This program is available through the Tennessee Department of Disability and Aging (DDA).
CLS services are individualized based on the needs of each resident and specified in the person-centered plan of care. Services may include hands-on assistance with activities of daily living (ADLs), supervision, transportation, and other day-to-day supports intended to help the client integrate seamlessly into their community such as:
- Assistance in improving skills needed for activities of daily living or assistance with activities of daily living as needed, such as bathing, dressing, personal hygiene and grooming, eating, toileting, transfer, and mobility.
- Assistance in improving skills needed for instrumental activities of daily living or assistance with instrumental activities of daily living as needed, such as household chores, meal planning, shopping, preparation and storage of food, and managing personal finances.
- Transportation to any and all appointments and community events. We are insured to transport clients.
- Building and maintaining interpersonal relationships with family and friends.
- Pursuing educational goals and employment opportunities.
- Participating fully in community life, including faith-based, social, and leisure activities selected by the individual.
- Providing all necessary support needed to successfully allow clients to live as independent as possible.
All activities are focused toward community inclusion, are person centered and reflect the client’s preference.
