Personal Assistance
Our Services:
A service chosen by the person supported, providing personalized support that enables them to live in the community and setting of their choice, while promoting their independence, rights, and full inclusion. It also ensures each resident’s choices and rights are upheld.
Personal Assistance services shall be delivered in a manner that adheres fully with standards applicable to Home and Community Based Services (HCBS) settings delivered under Section 1915(c) of the Social Security Act, except as supported by the individual’s specific assessed need and documented in their Person-Centered Support Plan (PCSP).
All goals, objectives, and required support for each client shall be developed through the person-centered planning process and documented in the PCSP. These include opportunities to pursue competitive integrated employment, engage in community life, and manage personal resources, as applicable to the client’s needs and preferences.
Support may include direct assistance with activities of daily living as needed, as well as participation in and learning how to independently complete these activities. Examples include:
- bathing
- dressing
- personal hygiene
- eating/feeding assistance
- meal preparation (excluding the cost of food)
- toileting and incontinence care, and
- assistance with transfer and mobility.
Support may also include household chores essential to the client’s health and wellbeing, such as washing dishes, doing laundry, cleaning areas of the residence used by the client, and managing their budget (including assistance with personal funds, as appropriate).
Additionally, we are insured to transport clients. Support may involve supervising and accompanying clients to medical appointments, as needed, and assisting with personal errands like grocery shopping, picking up prescriptions, paying bills, or visiting the post office.
Support shall be provided in a manner which ensures each client’s rights of privacy, dignity, respect, and freedom from coercion or restraint, while fostering autonomy, independence, and initiative in life choices. Personal Assistance may also include medication administration as permitted under Tennessee’s Nurse Practice Act, and other ‘non-complex’ health tasks, as permitted by State law.
Personal Assistance is a service that is provided for the direct benefit of the person supported. It is not a service that provides direct assistance to the other members of the household (e.g., meal preparation or laundry), unless those members are also supported through the waiver residing in the same home (e.g., when 2 siblings in the home are both enrolled in the waiver).
Documentation of service delivery shall be maintained for each person supported and shall reflect the total number of shared units of service provided, and the tasks performed/assistance provided for that individual.
Clients receiving Personal Assistance services may choose to receive services in a shared living arrangement with others who require varying support levels, different waiver services, or who participate in distinct HCBS programs, as permitted in state licensure laws and regulations, provided there is a willing, qualified provider who can safely meet the needs of all residents.
Personal Assistance may be offered during day or night hours, as outlined in the PCSP.
